Medical Release and Authorization

As Parent and/or Guardian of the named camper, I hereby authorize the diagnosis and treatment by the attending medical professional, of the minor child, in the event of a medical emergency, which in the opinion of the attending medical professional, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the attending physician to proceed with any medical or minor surgical treatment, x-ray examination and immunizations for the named camper. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to the Gan Israel Limassol and its affiliates including Directors, Coaches, and Team Parents to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the registered season.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.

Consent of Photography and Video recording

As part of the commitment to providing a positive and enriching experience for all campers, Camp Gan Israel Limassol occasionally capture moments through photography and video recording. As Parent and/or Guardian of the named minor camper, I hereby grant full rights to use the images resulting from the photography/video filming, and any reproductions or adaptations of the images for fundraising, publicity or other purposes to help achieve the group’s aims.
This might include (but is not limited to), the right to use them in their printed and online publicity, social media, press releases and funding applications. Your child’s privacy is respected—we do not include personal information in images.

Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Camp Gan Israel Limassol during the selected camp time period. In exchange for the acceptance of said child’s candidacy by Camp Gan Israel Limassol, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Gan Israel Limassol and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions.

In case of injury to said child, I hereby waive all claims against Camp Gan Israel Limassol including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities and field trips planned. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

Informed Consent and Acknowledgement for education content

I hereby grant my informed and voluntary consent as a parent to allow my child, to receive religious education in accordance with the tenets of Judaism and Chabad teachings. This consent encompasses the study of sacred texts, prayers, rituals, and moral principles. I understand that religious education plays a vital role in shaping my child’s character, fostering spiritual growth, and instilling ethical values. I consent to my child participating in religious classes, attending synagogue services, and engaging in age-appropriate discussions related to faith and tradition.

The curriculum may include, but is not limited to, Hebrew language instruction, Torah study, Jewish history, and Chabad-specific teachings. I authorize my child’s exposure to religious texts, including the Torah, Talmud, and other relevant literature. I trust the educators and religious leaders responsible for my child’s religious education to provide accurate, age-appropriate information. I expect them to create a nurturing and respectful environment that encourages questions, critical thinking, and spiritual exploration.

I permit my child to participate in religious ceremonies, such as Shabbat observance, Jewish holidays, and lifecycle events. My child may be involved in communal activities, including prayer services, holiday celebrations, and mitzvah projects.

I retain the right to withdraw this consent at any time by providing written notice to the educational institution or religious organization involved.

Confirmation

By clicking the “Confirm” button, I acknowledge and agree that I am providing an electronic signature, which will have the same legal effect as an original manual paper signature. This electronic signature will be equally binding for all the terms and conditions stated above, and I confirm that all the details provided are true and correct to the best of my knowledge.
